So, this is more or less exactly what finally happened. Blizzard changed the way Deathwing's fire was programmed so that if he already torched an area and you wandered into the fire, you would still get the achievement...as opposed to the previous mechanic of needing to be shot directly in the face by the inglorious bastard. Kestulefa was not killed (or even dismounted) when she got the achievement: she just swooped through a flame, took a minor tick of damage, and will now proceed to feel let down for the rest of her natural existence.
